Many organizations provide life insurance and other benefits for their
members. In the event that the deceased belonged to several organizations,
it may be beneficial for you to inform them of the death and inquire about
benefits to which you are entitled.
The following are sample letters that may help you when contacting these
organizations. On each letter, be sure to include the date, your full
name, address, phone number, and email address. Also, be sure to keep
a copy of each letter for your files.
